We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will be responsible for providing direct support to clients, developing individualized service plans, and coordinating resources to help clients achieve their goals. This role requires strong interpersonal skills, a deep understanding of the challenges faced by individuals experiencing homelessness, and a commitment to advocacy and social justice.

Job overview:

  • Hiring method: Contract

  • Location: 100% onsite near Broadview, Case Manager is required to travel to clients' homes in Greater Chicago Area (mileage reimbursement provided)

  • Hours: Monday - Friday, 9am-5pm (37.5 hours/week)

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop individualized service plans.

  • Provide case management services, including crisis intervention, counseling, and advocacy.

  • Coordinate and facilitate access to community resources, such as housing, healthcare, employment, and education.

  • Maintain accurate and up-to-date client records and documentation.

  • Participate in team meetings, training sessions, and professional development opportunities.

Qualifications include:

  • Associate or bachelor degree in social services or related field

  • 6 months or more of experience working in Case Management

  • Fluency in English and other languages (preferred)

  • Must have valid drivers license and reliable transportation

  • Must be comfortable fulfilling the contract through to completion

Pay Details: $21.00 to $24.00 per hour
