Case Manager – Chicago
LHH is hiring multiple Case Managers for our client based in Chicago! In this role, you will travel to client residences or group homes to provide service coordination for those with developmental disabilities. This is a fully onsite, contract to hire position.

Employment type: Contract to Hire
Location: Main office based near O’Hare airport; client locations vary throughout Cook and Lake counties
Hours: Monday – Friday, 830AM-5PM
Hourly pay rate: $24/hour

Required education, skills and experience
Bachelor degree in human services field or related
1 year of experience working with individuals with developmental disabilities
Reliable transportation and ability/willingness to travel to client homes and residences
QIDP certification or willingness to obtain (training and materials provided by client)
Responsibilities include,
Providing case coordination for clients, including conducting annual home visits for initial assessment and redeterminations
Conducting outreach to referrals received from community partners
Documentation of intake, discovery, personal goals and plans, and consent forms
Pay Details: $24.00 to $25.00 per hour
