Position Overview
The Case Manager provides client-centered case management services to unaccompanied, undocumented minors and their families. This role supports stabilization, access to resources, and coordination with internal programs and external partners, including legal service providers.
Key Responsibilities
Conduct client intakes, assessments, and develop individualized, client-centered service plans
Provide case management support focused on education, healthcare, family needs, and community resources
Deliver psychoeducation and coping skills support to youth and adolescents
Assess client safety and stability using child welfare principles through interviews, phone/video calls, and occasional home visits
Maintain accurate, timely documentation in accordance with program policies and reporting requirements
Coordinate referrals, advocacy, and follow-up services to ensure continuity of care
Collaborate with legal teams to support client cases and appropriately escalate concerns
Maintain communication with internal teams and external stakeholders
Participate in outreach activities and community events as needed
Manage a caseload of approximately 30-35 clients
Required Qualifications
Minimum 1 year of experience in case management or related social services work
Experience working effectively in multicultural environments with sensitivity and respect
Knowledge of trauma-responsive service provision and crisis intervention principles
Strong verbal and written communication skills
Ability to adapt in a fast-changing, client-focused environment
Experience working with youth populations
Background in child welfare, behavioral health, family services, or social services
Experience collaborating with legal or advocacy organizations

Pay Details: $25.00 to $26.00 per hour
