HEDIS Coordinator

The HEDIS Coordinator supports quality improvement initiatives by conducting outbound outreach to health plan members, assisting with medical records retrieval, and helping close HEDIS and risk adjustment gaps in care.
Key Responsibilities
Conduct outbound phone outreach to members for HEDIS, wellness, and quality initiatives
Support closure of care gaps including preventive care, medication adherence, and health risk assessments
Collect and track medical records from provider offices and internal systems
Coordinate outreach and retrieval efforts with provider offices (physician offices, IPAs, MSOs)
Document all outreach activity accurately in internal systems
Limited on-site provider office visits as needed (approximately 10–15%)
Participate in ongoing training related to HEDIS, Stars, and Risk Adjustment programs
Requirements
Native or near-native fluency in a required language (professional level in Korean, Farsi or Vietnamese)
Healthcare experience required (medical records, provider office, front desk, dental office, or similar)
Strong verbal communication skills (role is very phone-heavy)
Comfortable with outbound calling and documentation
EMR/CRM experience preferred
Valid driver’s license, reliable transportation, and ability to travel locally
A language assessment is required as a prerequisite to moving forward in the interview process.
Work Environment
Hybrid role with field?based responsibilities
Local travel to provider offices required
Ability to sit for extended periods and perform computer and phone work
Ability to lift up to 15 lbs as needed

Pay Details: $20.00 to $25.00 per hour
