Are you passionate about making a difference in mental health? As a Substance Use Disorder (SUD) Case Manager you'll offer support to patients dealing with behavioral health issues compounded by environmental and personal challenges. Collaborate as part of the patient care team, contribute to care plans, and inform medical staff about psycho-social issues affecting health conditions.
You'll support skill-building, assist patients in setting personal goals, and champion recovery principles.
Continuous follow-up, motivational interviewing, and goal setting are essential components of your work.
A Bachelor's degree in a related field is required.
Certification from a recognized certifying organization, agency, or Board demonstrating training and expertise in areas relevant to SUD services (such as CAADAC, CCAPP, CADTP, CADDE, CARR). required
At least 2 years of experience providing SUD services in a community and/or outpatient setting.
Location: Onsite in Detroit, MI 48209
Pay Details: $23.00 to $25.00 per hour
