LHH Recruitment Solutions is currently seeking a Case Manager for a growing organization in Long Beach, CA. As a Case Manager, you will play a vital role in optimizing patient outcomes and improving the overall healthcare experience by coordinating care, advocating for patients, and ensuring continuity.
Responsibilities
Provide community resources and housing referrals
Meet with clients every day to ensure they are provided with mental health resources, job sourcing, housing stability, etc.
Maintain accurate and up-to-date records of patient assessments, care plans, interventions, and outcomes.
Qualifications
Bachelor’s degree required
1+ year of Customer Service experience
Passion for working with the homeless populations
De-Escalation and conflict resolution skills
