Case Manager – Homelessness Services
Location: Oakland, CA
Pay Rate: $25–$26 per hour
Job Type: Contract
A mission-driven organization in Oakland is seeking a compassionate and resourceful Case Manager to support individuals experiencing homelessness. This is a contract opportunity ideal for someone who thrives in a fast-paced, community-focused environment and is committed to helping others achieve stability and independence.
Position Summary:
The Case Manager will work directly with clients to assess needs, develop individualized service plans, and connect them with critical resources such as housing, healthcare, and employment support. This role requires strong interpersonal skills, a trauma-informed approach, and a deep understanding of the challenges faced by unhoused populations.
Key Responsibilities:
  • Conduct client intake assessments and develop tailored service plans
  • Coordinate access to housing, medical care, mental health services, and job training
  • Advocate for clients with service providers and community partners
  • Maintain accurate and timely documentation in compliance with program standards
  • Monitor client progress and adjust plans as needed to support long-term success
Qualifications:
  • Minimum of 1–2 years of experience in case management, social work, or a related field
  • Familiarity with housing-first models and local social service systems
  • Strong communication, organizational, and problem-solving skills
  • Ability to manage a diverse caseload and work independently
  • Bachelor’s degree in Social Work, Psychology, or a related field preferred
  • Bilingual (English/Spanish) is a plus


Pay Details: $25.00 to $26.00 per hour
