Job Title: Case Manager (Homeless Services)
Location: Berkeley, CA
Salary: $23.00 - $29.00 per hour

Position Overview:
The Case Manager will be responsible for providing personalized support to individuals and families experiencing homelessness. You will work directly with clients to assess their needs, develop action plans, and connect them with the necessary resources and services to achieve housing stability and self-sufficiency.
Key Responsibilities:
Client Intake and Assessment: Conduct thorough assessments to understand clients' needs, strengths, and barriers to housing stability. Develop individualized service plans based on assessment outcomes.
Resource Coordination: Assist clients in accessing a range of services, including emergency shelters, transitional housing, financial assistance, job training, and healthcare. Provide guidance on navigating complex systems and completing necessary paperwork.
Case Management: Maintain regular contact with clients to monitor progress, provide support, and make adjustments to service plans as needed. Address any challenges or barriers that arise in the process of achieving housing stability.
Advocacy: Advocate on behalf of clients with service providers, landlords, and other stakeholders to ensure access to resources and resolve any issues related to housing and support services.
Documentation: Maintain accurate and confidential records of client interactions, service plans, and progress reports. Ensure compliance with agency policies and reporting requirements.
Collaboration: Work closely with other team members, community organizations, and government agencies to coordinate services and ensure a holistic approach to client support.
Community Engagement: Build and maintain relationships with local service providers, community organizations, and other stakeholders to enhance resource availability and support for clients.
Qualifications:
Experience: At least 1-2 years of experience in case management, social services, or a related field. Experience working with homeless populations or in a similar setting is preferred.
Skills: Strong interpersonal and communication skills, with the ability to work effectively with diverse populations and build rapport with clients.
Knowledge: Understanding of homelessness issues, local resources, and social services. Familiarity with housing systems and Fair Housing laws is a plus.
Organizational Abilities: Excellent organizational and time management skills, with the ability to handle multiple cases and prioritize tasks effectively.
Empathy: Demonstrated compassion and a non-judgmental approach to working with vulnerable populations.
Technology: Proficiency with Microsoft Office Suite and experience using case management software or databases.
