As an Integrated Care Coordinator, you'll play a pivotal role in conducting patient outreach, engagement, and psychosocial assessments for those with complex care needs. You'll be the driving force behind our Enhanced Care Management (ECM) program, developing patient-centered care plans and coordinating vital service referrals and delivery.

Responsibilities:
Connect with patients via phone and in-person at community and residential sites to build effective relationships.
Develop and implement patient-centered care plans, ensuring alignment with their values and goals.
Provide specialized housing navigation services, assisting patients in accessing and retaining public benefits, insurance, and housing.
Participate in team meetings, provide training and onboarding support, and assist with caseload triaging.
Lead crisis response efforts, including de-escalation and coordination with local mental health departments.
Qualifications:
A passion for working directly with low-income individuals from diverse backgrounds in a culturally responsive manner.
At least 3 years of experience in community-based healthcare or social work, or 1 year as a Case Manager II or III with certification as a Certified Alcohol and Drug Counselor.
Strong organizational, communication, and problem-solving skills, with the ability to adapt to change while maintaining a positive attitude.
High School diploma or GED required; Bachelors Degree in Social Work, Health, or Human Services preferred.

Compensation: $28-32/hr.
