Case Manager – Chicago, IL

LHH Recruitment Solutions is seeking a Case Manager to join a dedicated team in Chicago, IL. This full-time role is Monday through Friday, 8:30 AM – 4:30 PM. Candidates must have a Bachelor’s or Master’s degree and be willing to travel to clients’ homes (MUST possess valid Driver’s License and reliable transportation).

Key Responsibilities:

• Provide case management and supportive services to help clients and families achieve self-sufficiency

• Develop, track, and assess client progress toward goals

• Advocate for clients in securing financial assistance, benefits, housing, education, childcare, job training, and treatment programs

• Conduct psycho-social assessments and client evaluations in homes, agency properties, or other accessible locations

• Maintain accurate and up-to-date client records and databases in compliance with agency policies and regulatory standards

• Organize and lead monthly support group meetings, create service plans, and provide necessary interventions

• Prepare routine and ad-hoc reports as needed

• Stay informed and comply with laws and regulations relevant to the role and services provided

• Participate in community outreach initiatives to connect clients with available resources

• Meet with clients as required by program guidelines or based on individual needs
