Case Manager
Location: Onsite – Chicago, IL 60007
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Pay Rate: $23 - $25 per hour
Job Description:
We are seeking a dedicated Case Manager to join our team and provide essential support to individuals experiencing homelessness. This role focuses on assisting clients in accessing resources, securing stable housing, and developing individualized service plans to promote self-sufficiency. The ideal candidate will be passionate about advocating for vulnerable populations and have experience in case management, social services, or a related field.
Key Responsibilities:
Conduct assessments to identify client needs and develop individualized care plans
Assist clients in obtaining housing, employment, healthcare, and social services
Provide crisis intervention, advocacy, and referrals to appropriate resources
Maintain accurate and timely documentation of client interactions and progress
Collaborate with community partners, service providers, and government agencies
Monitor and evaluate client progress, adjusting service plans as necessary
Educate clients on life skills, financial management, and available support programs
Ensure compliance with all policies, regulations, and confidentiality standards
Qualifications:
Bachelor’s degree preferred in Social Work, Psychology, Human Services, or a related field
Minimum of 2 years of experience working with the homeless population
Strong understanding of trauma-informed care and case management best practices
Ability to effectively communicate and build rapport with diverse populations
Proficiency in documentation, record-keeping, and navigating community resources
Knowledge of local housing programs, public assistance, and social service systems
Ability to work independently and as part of a multidisciplinary team
Strong organizational, problem-solving, and crisis intervention skills
Pay Details: $23.00 to $25.00 per hour
