We’re looking for empathetic, driven Bilingual Case Manager in Regions E & F of Chicago. You’ll walk alongside pregnant individuals—many of whom are navigating complex challenges such as health issues, financial instability, addiction, or domestic violence—from pregnancy through their baby’s first 6 months.

Great opportunity for recent graduates who completed their internship in social services or in human services.

What You’ll Do:

  • Build trust and provide 1:1 support to pregnant and postpartum individuals.

  • Help clients access vital services: medical care, housing, food, transportation, and more.

  • Meet clients where they are—literally. Whether it’s a phone call, a visit to their home, a local library, you’ll be there providing support.

  • Manage a caseload of 225–250 clients, connecting with each 3–6 times throughout their journey.

  • Promote maternal and infant health, safe sleep, mental wellness, and developmental milestones.

  • Collaborate with community partners and attend outreach events to grow program awareness.


What You Bring:

  • Associate degree required in social work, public health, or related field. Bachelor’s degree preferred.

  • A minimum of 1 year of experience in social work preferrably working with pregnant women and/or young children.

  • A heart for advocacy, cultural humility, and trauma-informed care.

  • Strong communication skills and the ability to work independently.

  • Familiarity with Illinois community resources and public benefit programs is a plus.

  • Must have reliable transportation to meet clients, have a valid driver license, and pass background check.

  • Bilingual in any language Spanish, Polish, Chinese (including Mandarin and Cantonese), and Tagalog.

Pay Details: $20.00 to $23.00 per hour
