Pay: $21-28/hr


Hours: Days, Mon-Fri


Position Overview: The Case Manager will play a crucial role in helping clients overcome homelessness. This position involves guiding clients through various stages of homelessness, from street outreach and interim housing to transitional and permanent housing. The Case Manager will provide personalized support to ensure clients access and maintain essential services, including healthcare, social services, and community resources.


Key Responsibilities:



  • Client Support: Assist clients through all phases of homelessness, including street outreach, interim housing, transitional housing, and permanent housing. Provide individualized support to help clients address their strengths, barriers, health needs, and housing goals.


  • Service Coordination: Help clients navigate healthcare, social services, and support systems. Develop and implement a comprehensive care plan that includes housing and health outcomes.


  • Assessment & Planning: Conduct intake assessments and create Individualized Service Plans (ISP) addressing housing goals, income maintenance/increase, and other personal objectives such as medical, mental health, substance use, financial resources, vocational, and social support needs.


  • Resource Connection: Provide referrals and link clients to resources that support their ISP goals. Complete program-specific assessments (e.g., VI-SPDAT) and perform a 90-day review to evaluate progress and adjust plans as needed.


  • Documentation & Compliance: Maintain accurate and timely case file notes and client records in accordance with program and organizational policies. Ensure all required documents are current and complete.


  • Collaboration & Teamwork: Work effectively with colleagues, participants, and stakeholders to achieve team goals. Foster positive working relationships and recognize the contributions of others.


  • Communication: Clearly and effectively convey information to individuals and groups, both verbally and in writing, to ensure understanding and engagement.


  • Critical Thinking & Problem Solving: Analyze and evaluate information to make informed decisions that achieve desired outcomes. Address issues with logical reasoning and sound judgment.


  • Ethics & Boundaries: Uphold ethical and professional standards, demonstrating honesty and integrity while maintaining professional boundaries.


  • Driving: Travel as needed to meet clients and access resources is an essential function of this position.


Qualifications:



  • Proven experience in case management or a related field.


  • Strong understanding of homelessness and the services required to address it.


  • Excellent communication and interpersonal skills.


  • Ability to work collaboratively in a team environment.


  • Proficient in critical thinking and problem-solving.


  • Commitment to maintaining ethical standards and professional boundaries.


  • Valid driver’s license and reliable transportation.


Preferred Skills:



  • Familiarity with local resources and support systems in Costa Mesa, CA.


  • Experience with program-specific assessments and documentation.
