LHH Recruitment Solutions is hiring a Case Manager Medical Scheduler for a healthcare client. This is a 3-6 month contract position, fully remote, supporting the Pacific Time Zone. Candidates must reside Nevada and Oregon only. This for the the Transition of Care for ambulatory patients. Discharge experience is preferred but not required.
Position Details:
Hours: 10:00 AM - 6:30 PM PST
Weekends: Rotational every few weeks
Holidays: Required
Contract Duration: 3-6 months
Key Responsibilities:
Transition of Care Coordination: Manage and implement transition of care (TOC) and discharge (DC) plans for ambulatory patients. Work with Case Managers and Social Workers to assess patient needs, develop TOC plans, and ensure timely discharge.
Communication and Documentation: Maintain accurate documentation in the electronic medical record. Communicate effectively with patients, families, care providers, and external vendors.
Collaboration: Work closely with the Care Coordination (CC) team to monitor progress, identify barriers, and ensure TOC goals are met. Participate in daily team huddles and department meetings.
Customer Service: Build positive relationships with all customers and partners. Use effective communication skills to resolve issues and provide constructive feedback.
Requirements:
Education: High School Diploma or equivalent education/experience.
Experience: Minimum of 1-year recent relevant experience.
Skills and Knowledge:
Proficiency with Epic EHR is essential.
Strong oral and written communication skills.
Excellent interpersonal and time management skills.
Ability to work independently and prioritize tasks effectively.
Intermediate PC and word processing skills.
Ability to work in a fast-paced environment with shifting priorities.
Additional Information:
Candidates must be open to rotating weekends and working holidays.
The role involves handling a high volume of faxes and voicemails, data entry, and supporting UR transmission.
Pay Details: $20.00 per hour
