possible to work from home 1-2 days per week after training is complete.
Traveling 1-2 days per month to either Ft Wayne or Anderson (mileage is reimbursed)
This position requires travel to patient homes, schools, and outreach clinics.
Schedule: 8-4:30 30 minute lunch
Hours can vary depending on outreach clinic days

Job Overview:
LHH Recruitment Solutions has partnered with a healthcare organization seeking a Social Worker. This position is based in Indianapolis, IN and requires on-site presence & occasional travel. The Social Worker is responsible for protecting and improving the social and mental well-being and functioning of patients and families. This is done by assessing patient and family needs and connecting them with community based social services and resources.
Responsibilities:
Participate in comprehensive clinics to assess the psychosocial needs of patients and families.
Collaborate with multidisciplinary team members to establish an appropriate plan of care .
Consult with providers during patient office visits to provide psychosocial services.
Educate patients and families about available community resources.
Serve as an advocate; coordinating and connecting patients with services and assistance programs.
Provide therapeutic psychosocial assessment, supportive counseling, crisis intervention, monitoring via telephone, email, texting, virtual, and in person interaction in the home, office, or hospital.
Contribute to discharge planning process for hospitalized patients.
Participate in company events.
Monitor legislation impacting person with chronic disorders.
Coordinate with billing and insurance departments to facilitate resolution of related issues and navigating Medicaid, Medicare, and disability systems, including application process.
Qualifications:
3 years of related experience in healthcare setting is required.
Master’s Degree in social work required – open to candidates currently in Master’s program.
Valid Licensed Clinical Social Worker (LCSW) in state of Indiana, strongly preferred; Valid licensed Social Worker considered if currently pursuing clinical licensure.
Valid Driver’s License and auto insurance is required.
