- Conduct weekly one-on-one case management meetings with clients, making recommendations for additional services as needed.
- Monitor client files to ensure accuracy, quality, and compliance with grant requirements.
- Maintain client confidentiality in line with policies and funding regulations.
- Assist individuals and families with issues related to permanent housing, social services, mental health, medical concerns, domestic violence, and parenting, as needed.
- Identify and recommend appropriate community resources and services for clients.
- Evaluate clients' readiness for transition, formulate service plans and goals, and support clients in implementing these plans.
- Attend internal and external training to stay current with changes and best practices.
- Participate in Multidisciplinary Case Planning and Supervision Meetings.
- Perform other duties as assigned by supervisors or directors.
- Bachelor’s degree required.
- Ideally, 1+ years of experience in a related field.
- Experience working with homeless populations, substance abuse issues, or facilitating groups is preferred.
- Strong communication and organizational skills.
- Schedule: Monday to Friday, 8:00 AM - 4:30 PM
- Location: SPA 5 - Los Angeles, CA
- Job Type: Full-time, Temp-to-Hire
- Pay: $24.00 per hour
- Start Date: ASAP
