Case Manager LHH is searching for a dedicated candidate to provide comprehensive, strength-based case management services to individuals and families in need. Our client’s mission is to assist clients in their transition to and maintenance of permanent housing, ensuring they receive the support necessary for long-term success. This role involves working directly with clients, monitoring their progress, and making recommendations for additional services as needed. Key Responsibilities:
  • Conduct weekly one-on-one case management meetings with clients, making recommendations for additional services as needed.
  • Monitor client files to ensure accuracy, quality, and compliance with grant requirements.
  • Maintain client confidentiality in line with policies and funding regulations.
  • Assist individuals and families with issues related to permanent housing, social services, mental health, medical concerns, domestic violence, and parenting, as needed.
  • Identify and recommend appropriate community resources and services for clients.
  • Evaluate clients' readiness for transition, formulate service plans and goals, and support clients in implementing these plans.
  • Attend internal and external training to stay current with changes and best practices.
  • Participate in Multidisciplinary Case Planning and Supervision Meetings.
  • Perform other duties as assigned by supervisors or directors.
Qualifications:
  • Bachelor’s degree required.
  • Ideally, 1+ years of experience in a related field.
  • Experience working with homeless populations, substance abuse issues, or facilitating groups is preferred.
  • Strong communication and organizational skills.

  • Schedule: Monday to Friday, 8:00 AM - 4:30 PM
  • Location: SPA 5 - Los Angeles, CA
