LHH Recruitment Solutions is searching for a full-time Case Manager on a contract to hire basis for a growing client in Los Angeles, CA and Long Beach, CA. Our client is seeking individuals that are highly dynamic and adaptable, empathetic, and committed to addressing the unique challenges faced by their clientele.
Responsibilities:
Conducting comprehensive assessments to determine the needs, strengths, and barriers of individuals or families seeking assistance.
Interviewing clients, reviewing their history, and evaluating their immediate and long-term needs.
Collaborating with clients to develop individualized case plans that outline short-term and long-term goals.
Identifying and connecting clients with appropriate resources.
Coordinating with other service providers and community organizations to ensure clients receive the necessary support.
Serving as an advocate for clients, ensuring their rights are protected, and helping them navigate complex systems.
Providing immediate assistance and support during crises.
Offering guidance and training to enhance clients' independent living skills.
Regularly checking in with clients to monitor their progress, assess ongoing needs, and make necessary adjustments to the case plan.
Maintaining accurate and confidential case records, including client assessments, case plans, progress notes, and other required documentation.
Qualifications:
Bachelors Degree, required.
A minimum of 1+ years of experience in related field, required.
Proficient with Microsoft Office, required.
Reliable transportation and valid drivers license, required.
Compensation/Benefits: $23-$25hr
Job Type: Contract to hire
Schedule: Monday - Friday, 8am - 5pm
Pay Details: $23.00 to $25.00 per hour
