Case Manager

Compensation: $24/hour

Must hold a completed Bachelor degree, and 1-2 years in direct Case Management services working with the Homeless populations.

ONSITE Locations: South Los Angeles area

• Provide case management services, including client assessment, goal-setting, and individualized service planning.

• Maintain accurate and timely documentation of client interactions and progress.

• Coordinate and facilitate access to resources such as housing, employment, medical, and mental health services.

• Engage in crisis intervention and support as needed.

• Collaborate with other professionals and community partners to ensure comprehensive care.

• Conduct follow-up and outreach to clients in various community settings.

Qualifications:

• Minimum of 1-2 years of Case Management or related experience.

• Required: Must hold a completed Bachelor Degree.

• Knowledge of community resources and the ability to navigate social service systems.

• Strong communication and organizational skills.

• Ability to work effectively both independently and as part of a team.

• Familiarity with trauma-informed care practices is preferred.

• Bilingual skills (Spanish, Russian, or other) are a plus.
