Case Manager
Compensation: $24/hour
Must hold a completed Bachelor degree, and 1-2 years in direct Case Management services working with the Homeless populations.
ONSITE Locations
: South Los Angeles areaSchedule: Full-Time, M-F 8am - 4:30p
Key Responsibilities:
Provide case management services, including client assessment, goal-setting, and individualized service planning.
Maintain accurate and timely documentation of client interactions and progress.
Coordinate and facilitate access to resources such as housing, employment, medical, and mental health services.
Engage in crisis intervention and support as needed.
Collaborate with other professionals and community partners to ensure comprehensive care.
Conduct follow-up and outreach to clients in various community settings.
Qualifications:
Minimum of 1-2 years of Case Management or related experience.
Required: Must hold a completed Bachelor Degree.
Knowledge of community resources and the ability to navigate social service systems.
Strong communication and organizational skills.
Ability to work effectively both independently and as part of a team.
Familiarity with trauma-informed care practices is preferred.
Bilingual skills (Spanish, Russian, or other) are a plus.
