The Case Manager works in the interim housing setting to provide guest case management services. Case Managers collaborate with other departments internally as new guests are admitted into Interim Housing. The Case Manager assists guests with building skills, reducing mental health and functional barriers to obtaining and maintaining housing, and strengthening community social service linkages necessary for the guest to integrate into the community once they leave interim housing entirely.
  • Conduct and complete all new guest intakes in coordination with guests and enter information into HMIS promptly and within the documentation standards.
  • Conduct a full VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) at admission for all new guests to aid in the Individual Service Plan (ISP) development.
  • Collaborate with other team members and guests to develop Individualized Service Plans (ISP) that address barriers to obtaining services, housing, and appropriate health care needs.
  • Meet with assigned guests a minimum of 1x/week to support with progression towards goals identified by guest in their ISP.
  • Provide ongoing case management support with benefits establishment (GR, SSI, TANF, Veterans pension, etc.), obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals and health related assistance as identified on the ISP.
  • Provide referrals and linkages to primary and behavioral health services and other supportive services.
  • Collaborate with referring organizations to coordinate guests’ care and ensure guests receive the services and support they need to achieve and maintain health, mental health, and housing stability.
  • Assist with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing.
  • Assist and educate guests about care and safety regarding their animals as needed.
  • Advocate with prospective landlords and identify appropriate permanent housing options for guests.
  • Maintain documentation standards as set forth by the program contract and PATH policies, including maintaining compliance with HIPAA policies and practices.
  • Complete progress notes on every face to face or telephone contact with guest or collateral contact by the next business day at 5 pm.
Qualifications
  • Experience in healthcare, case management, homeless services, or related field preferred but all experience is welcomed
  • Experience with HMIS
  • Ability to work flexible hours, which may include evenings or weekends.
