LHH is hiring an experienced Medical Case Manager for our client in Manhattan! In this role, you will be responsible for client intake, assessment, and care plan development. Individuals with an Associate or Bachelor degree and 1-3 years of experience are encouraged to apply!

Position details:

• Employment type: Direct Hire / Permanent

• Location: 100% onsite - Manhattan (NY)

• Hours: Monday - Friday, 930AM-6PM; occasional weekend hours for events

Responsibilities:

• Providing case management to clients needing medical care and other services

• Performing client intake, assessments, reassessments and follow ups

• Individualized care plan development and reassessment

• Liaising between PCP and other clinicians

• Conducting 16 telephone encounters per day

Qualifications:

• Bachelor degree in human services, social work, psychology or related field and 1 year of Case Management experience

• OR Associate degree in aforementioned fields and 3 years of Case Management Experience

• Experience with eClinical Works is preferred
