$27.00 - $29.00 per Hour
- Outreach & Engagement: Conduct outreach and build relationships with patients via phone and in-person at various sites.
- Case Management: Develop and implement patient-centered care plans, manage caseloads, and coordinate referrals and services.
- Housing Navigation: Assist with document readiness, housing problem-solving, and assessments for the Coordinated Entry System.
- Health Education: Provide training on harm reduction and disease risk-mitigation.
- Crisis Intervention: Lead crisis response, de-escalation, and follow-up care.
- Documentation: Maintain accurate records and document patient interactions in required data systems.
- Education: High School diploma or GED required. Bachelor’s Degree in Social Work or related field preferred but not required.
- Experience: At least three years in community-based health or social work; bilingual in English/Spanish required.
- Skills: Strong organizational, problem-solving, and interpersonal skills. Proficiency in Microsoft Office and database management.
