Case Manager
LHH Recruitment Solutions is hiring a Case Manager for our healthcare client located in the Oakland Area. This role will be a blend of field and in-office administrative work and will be a contract to hire opportunity. This case management role will focus on assisting patients with their care plans and healthcare needs, as well as other supportive services including (but not limited to) housing, behavioral health, and documentation.
Interested candidates must feel comfortable working with clients who are experiencing complex health ailments including homelessness, mental health, and/or substance abuse ailments.
Requirements
High School Diploma or equivalent required, Bachelor's Degree in Psychology or Social Service strongly desired
Minimum 2 years case management experience
Proficiency within Microsoft Office and comfortability learning different software systems
Valid Driver’s License required as this role is field work based
Skills
Compassion and empathy for helping others
Excellent written and verbal communication skills
Ability to work well in a high-pressure enviornment and can adapt in a fast-paced when needed
Understanding of community resources that will best serve clientele
Can de-escalate situations as needed in calm and collected manner
Strong ability to connect with a wide array of individuals
Pay Details: $25.00 to $30.00 per hour
