LHH Recruitment Solutions is hiring a Case Manager for our healthcare client located in Oakland. This will be a Monday-Friday, onsite position and a contract opportunity. Our client is urgently hiring for this role!

This position will be focused on supporting clients with linkages to primary health, behavioral health, and other community resources. Interested candidates must feel comfortable working in a fast-paced environment and working with clients and external community contacts in order to help individuals along their ISPs.

• High School Diploma or equivalent, Bachelor's Degree strongly desired
• Minimum 2 years case management/social services experience
• Bilingual English/Spanish strongly desired, not required

• Strong ability to multitask and thrive in fast-paced environments
• Able to work well with a wide array of individuals and communicate accordingly
• Excellent ability to prioritize and meet deadlines
• Can de-escalate situations as needed and remain calm in high-pressure situations
• Excellent attention to detail and accuracy
• Proficient working within multiple software systems including Microsoft Office and HMIS
