Location: West Oakland
Duration: Temp to Hire
Hours: Monday-Friday, 8:30 AM - 5:00 PM (some flexibility required)

Software: EPIC & HMIS preferred
Requirements: Valid Driver’s License & Reliable Transportation

Health Screenings Required:

• 7-year background check & TB Skin Test upon offer

• Flu shot required (or must wear a mask from November to March)

• COVID vaccine + booster unless the most recent COVID vaccine was the first

Job Description:

The Case Manager is responsible for providing services to patients with complex care needs. This role focuses on patient outreach, engagement, and psychosocial service assessment, assisting in the development of patient-centered care plans.

Responsibilities:

• Conducts patient outreach and engagement for individuals with complex health and social service needs

• Assesses psychosocial needs and helps develop patient-centered care plans

• Coordinates service referrals and delivery, ensuring patients receive necessary support

• Meets clients in various settings, including homes, clinics, hospitals, shelters, supportive housing sites, and encampments

• Provides case management outside of traditional healthcare settings, addressing social determinants of health

• Works closely with an interdisciplinary team to improve patient health outcomes and access to services
