LHH is seeking a dedicated Case Manager in Philadelphia, PA. This role involves providing essential support to residents in shelters, coordinating housing services, and helping clients transition to stable living environments.

Responsibilities:

• Offer supportive housing counseling and advocacy to shelter residents.

• Conduct individual case management, needs assessments, and action planning.

• Assist residents with appointments and applications for relocation fund assistance.

• Develop and distribute informational materials about available resources.

• Network with community resources to provide comprehensive support to clients.

Qualifications:

• Bachelor's Degree in social services or a related field (required).

• 2 years of direct service experience with domestic violence or underserved population.

• Experience in a residential setting and prior case management.

• Driver's License required.

• Proficiency in Spanish preferred.

• Must pass criminal (state & FBI) and child abuse clearances.

Schedule and Compensation:

Monday to Friday, 9:00 AM - 5:00 PM, Pay Rate: $19.50/hour
