Behavioral Health Case Manager
Contract location: Onsite in Phoenix, AZ 85021
Monday to Friday, 8:00 AM - 5:00 PM
Project Duration: Approximately 3-6 months
Pay Range: $19.52 - $27.38 per hour (dependent on a combination of BH experience and relevant education)
Responsibilities:
Conduct assessments to determine clients' needs and develop individualized care plans.
Coordinate and monitor services, including medical, psychiatric, social, and housing services.
Advocate for clients with SMI, Mental Health Diagnosis or Drug Addiction to ensure they receive appropriate and effective services.
Maintain detailed and accurate client records and documentation.
Facilitate group and individual counseling sessions as needed.
Provide crisis intervention and support to clients in emergency situations.
Collaborate with community resources and service providers to ensure comprehensive care.
Qualifications:
REQUIRED Minimum of 3 years in Behavioral Health Case Management working with SMI, Mental Health Diagnosis and Drug Addiction
REQUIRED Bachelor or Master’s degree in Behavioral Health or related field
Strong understanding of mental health diagnoses and treatment options.
Excellent organizational and time management skills.
Ability to work effectively both independently and as part of a team.
Strong communication and interpersonal skills.
Proficiency in electronic health records (EHR) systems is a plus.
Pay Details: $19.52 to $27.38 per hour
