Job Title: Behavioral Health Case Manager
Location: On-Site, Phoenix AZ (Openings in Tempe as well)
Hours: M – F 8a – 5p
Pay: $19.52/hr - 27/hr Depending on years of experience (Set tiers)
Job Description:
We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will have 2+ years of experience in behavioral health and a passion for supporting individuals on their recovery journey.
Responsibilities:
Provide individualized case management services to clients.
Develop and implement recovery plans in collaboration with clients and their support systems.
Conduct assessments and facilitate access to resources and services.
Monitor progress and adjust plans as needed to ensure successful outcomes.
Qualifications:

Bachelor’s degree in social work, psychology, or a related field preferred.
Minimum of 2 years’ experience in behavioral health or a related field.
Strong communication and interpersonal skills.
Ability to work collaboratively in a team-oriented environment.
