Job Title: Case Manager
Location: San Diego, CA
Pay: $21 - $26 per hour
Employment Type: Contract to Hire
Schedule: Overnight, 10:00 PM - 6:00 AM
Job Description: We are seeking a dedicated and compassionate Case Manager to join our team in San Diego, CA. As a third-party recruiter, we are looking for a candidate who excels in providing support and guidance to individuals in a shelter setting. Key Responsibilities:
  • Provide comprehensive case management services to clients in a shelter environment.
  • Develop and implement individualized care plans to address clients' needs and goals.
  • Coordinate with healthcare providers, social services, and community resources to ensure clients receive appropriate support.
  • Conduct regular assessments and monitor clients' progress.
  • Advocate for clients' rights and access to services.
  • Maintain accurate and confidential client records.
  • Facilitate group sessions and educational workshops.
Qualifications:
  • Previous experience in case management or a related field is preferred.
  • Strong understanding of issues faced by individuals in shelter environments.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in using case management software and tools.
  • Bachelor's degree in social work, psychology, or a related field is preferred.

Pay Details: $21.00 to $26.00 per hour
