Job Description: We are seeking a dedicated and compassionate Case Manager to join our team in San Diego, CA. As a third-party recruiter, we are looking for a candidate who excels in providing support and guidance to individuals in a shelter setting. This is a rewarding opportunity for someone passionate about making a positive impact in the lives of others.
Key Responsibilities:
- Provide comprehensive case management services to clients in a shelter environment.
- Develop and implement individualized care plans to address clients' needs and goals.
- Coordinate with healthcare providers, social services, and community resources to ensure clients receive appropriate support.
- Conduct regular assessments and monitor clients' progress.
- Advocate for clients' rights and access to services.
- Maintain accurate and confidential client records.
- Facilitate group sessions and educational workshops.
- Previous experience in case management or a related field is preferred.
- Strong understanding of issues faced by individuals in shelter environments.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in using case management software and tools.
- Bachelor's degree in social work, psychology, or a related field is preferred.
