Swing Case Manager


(ONSITE) Santa Ana, CA 92704


Schedule: (Swing) Tuesday - Saturday 3:30p – 11:30p


Pay: $23 per hour


Key Requirements:



  • Provide comprehensive case management services to underserved individuals.


  • Conduct intakes, and assessments to determine participants needs and develop individualized service plans (ISP), schedule interdisciplinary meetings.


  • Connect participants with appropriate resources and services to address barriers.


  • Collaborate with community partners, shelters, and other agencies to facilitate access to resources and coordinate appropriate care.


  • Maintain accurate and up-to-date documentation of participant interactions, progress, resources and referrals.


  • Advocate on behalf of participants to ensure access to essential services and support their goals for housing stability and self-sufficiency.


  • Participate in team meetings, case conferences, and ongoing training sessions as required.


Qualifications:



  • Minimum 1 year experience in Case Management, Social Work.


  • Comfortable working with homeless population.


  • Strong interpersonal skills and the ability to build rapport with diverse populations.


  • Excellent communication and problem-solving abilities.


  • Compassionate and non-judgmental approach to working with clients.
