Schedule: Monday - Friday, 8:00 AM - 5:00 PM (may vary based on clinic needs: 7:00 AM - 4:00 PM or 9:00 AM - 6:00 PM)
Job Description:
LHH is hiring for a dedicated Patient Care Coordinator in Temecula, CA. We are looking for motivated, team-oriented candidates who are passionate about delivering exceptional patient-centric care in an outpatient clinic. Responsibilities:
- Patient Care Duties (70% of Role):
- Enhance the patient experience through all touchpoints.
- Greet and check in patients upon arrival, following the intake procedure.
- Communicate compassionately and professionally with patients, referral sources, and fellow associates.
- Provide excellent customer service to all referral sources.
- Monitor new referrals and schedule daily appointments.
- Schedule patients' visits according to their Rx, authorization, or the Physical Therapist's recommendation.
- Follow the patient 'No Show' procedure.
- Prepare and process medical records requests within 48 hours.
- Retrieve and process mail/faxes daily.
About the Role:
This is a temp-to-hire position, offering you the opportunity to become a permanent member of the team based on your performance. As a Patient Care Coordinator, you will be critical in ensuring patients have a positive experience from their first point of contact to their final visit.
