The Case Manager will be responsible for managing a caseload of clients, providing them with the necessary support, resources, and guidance to improve their quality of life. The ideal candidate will have extensive experience in social work, counseling, or a related field, and will possess excellent communication and organizational skills.

Key Responsibilities

· Conduct comprehensive assessments of clients' needs and develop individualized care plans.

· Coordinate and monitor services provided to clients, ensuring their needs are met in a timely and effective manner.

· Advocate for clients and help them navigate complex systems to access necessary resources and services.

· Provide ongoing support and counseling to clients, helping them to set and achieve personal goals.

· Maintain detailed and accurate records of client interactions, progress, and outcomes.

· Collaborate with other professionals and agencies to ensure a holistic approach to client care.

· Participate in team meetings, training sessions, and professional development opportunities.

· Stay informed about relevant laws, regulations, and best practices in the field.



Pay Details: $22.00 to $25.00 per hour
