Case Manager (Direct Hire)
Location: Ventura, CA (Onsite)
Compensation: $24-$28/hour

About the Opportunity

LHH Recruitment Solutions is partnering with a growing organization in Ventura, CA, to hire a full-time Case Manager on a direct hire basis. We are seeking highly dynamic, empathetic, and adaptable professionals committed to addressing the unique challenges faced by our clientele.

Key Responsibilities:

• Conduct comprehensive assessments to identify the needs, strengths, and barriers of individuals or families seeking support.

• Interview clients to review their history and evaluate immediate and long-term needs.

• Collaborate with clients to create personalized case plans with clear short-term and long-term goals.

• Connect clients with appropriate resources and services.

• Coordinate with service providers and community organizations to ensure holistic support.

• Advocate for clients, safeguarding their rights and guiding them through complex systems.

• Provide immediate assistance and crisis support when necessary.

• Offer guidance and training to promote clients’ independent living skills.

• Conduct regular follow-ups to monitor client progress, reassess needs, and adjust case plans as needed.

• Maintain accurate and confidential records, including assessments, plans, progress notes, and documentation.

Qualifications:

• High school diploma or GED (required).

• Bachelor’s degree with 2+ years of relevant experience (required), or Master’s degree with 1+ year of healthcare-related experience.

• Proficiency in Microsoft Office (required).

• Reliable transportation and valid driver’s license (required).
