LHH Recruitment Solutions is searching for a full-time Case Manager Assistnant on a Direct Hire basis for a growing client in Ventura, CA. Our client is seeking individuals that are highly dynamic and adaptable, empathetic, and committed to addressing the unique challenges faced by their clientele.
This is an onsite position and the candidate needs to live in or near Ventura, CA.
Responsibilities:
Build community trust among residents and assess their needs, including special needs groups, to recommend service plans and referrals.
Provide case management services and develop a resource directory with local and accessible resources.
Organize social, educational, and recreational programs through partnerships with volunteers, local non-profits, government agencies, and community resources.
Develop and coordinate workshops focused on resident needs, including wellness, life skills, financial literacy, mental health, technology, and community building.
Establish collaborative relationships with property managers and CEDC departments for effective communication on resident activities, meetings, and special events.
Document program activities and maintain accurate records via program database metrics.
Conduct annual resident satisfaction surveys and prepare quarterly and annual reports to measure impact.
Conduct intakes, orientations, and needs assessments of residents to create customized service plans.
Plan and coordinate on-site educational activities on topics such as lease and house rules, independent living, health and wellness, home safety, and disaster preparedness.
Facilitate support groups and resident leadership activities, including the establishment of a Resident Council as needed.
Qualifications:
Minimum of two years of experience in social work, supportive services, case management, or a related field.
Bilingual in English/Spanish is preferred.
Strong background in working with low-income individuals, individuals with special needs, and those with mental, physical, or developmental disabilities.
Knowledge of community resources, services, and public benefits for low-income populations, veterans, and farm workers.
Group facilitation, conflict resolution, and mediation skills are preferred.
Ability to collaborate with staff, property managers, and residents in implementing assistance and programs.
Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)..
Compensation/Benefits: $21-$23/hr
