Position Overview: The Case Manager will work under the supervision of the Director of Resident and Community Services, supporting low-income households, previously homeless, disabled, and mentally ill residents. This role focuses on permanent supportive housing, providing case management services aimed at housing retention and stability. As a Case Manager, you will be responsible for connecting residents with community resources, managing behaviors that could interfere with housing success, and supporting residents' overall well-being and mental health. Key Responsibilities:- Provide on-site, direct assistance to residents experiencing housing crises or homelessness.
- Deliver intensive case management services, including psychoeducation, supportive counseling, crisis interventions, and housing readiness support.
- Collaborate with an interdisciplinary team to serve high-needs homeless/formerly homeless individuals with mental illness, substance abuse disorders, and other conditions.
- Conduct psychosocial assessments and create individualized case plans/treatment goals.
- Maintain client records and ensure compliance with HIPAA and other regulations.
- Coordinate services with the Housing Authority, Ventura County Behavioral Health, Veterans Administration, and other partners to support optimal housing outcomes.
- Facilitate access to resources such as benefits advocacy, food pantries, mental health services, and more.
- Meet with residents weekly to review progress, discuss tenant rights/responsibilities, and support successful tenancy.
- Assist clients in applying for housing assistance programs such as Section 8 and CalWorks.
- Strong understanding of social services, public assistance, healthcare, mental health, and other systems relevant to low-income and unhoused populations.
- Empathy and a non-judgmental approach when working with individuals in crisis.
- Ability to advocate effectively for vulnerable populations.
- Familiarity with Trauma-Informed Care, Motivational Interviewing, and Harm Reduction Models.
- Knowledge of fair housing laws and tenant relations.
- Ability to thrive in a fast-paced, mission-driven environment.
- Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
- Bilingual in English/Spanish preferred but not required.
- Bachelor’s degree in social work or a related field plus two (2) years of experience in social services, OR a master’s degree in social work or a related field with one (1) year of case management experience.
- Valid California driver’s license and proof of automobile liability insurance.
